Definition

Silvercare is a healthcare plan from Medybiz Private Limited, to provide home-based healthcare services to the elderly. The service can be availed by any individual who is 60 years or above or can be sponsored by any individual for another individual above 60 years of age.

Availability

The service is currently available in Mumbai.

Applicability

  1. The person who buys Silvercare is called the Sponsor and the person who avails it  is the beneficiary. 
  2. The beneficiary can avail all the services of Silvercare Package only in the city selected at the time of purchase by the sponsor and he is not allowed to avail the services under this plan at any other cities where the Silvercare Service is available.

Eligibility

  1. Membership is open only to Beneficiaries who are residing in India, at the time of enrollment and would continue to do the same throughout the period of program tenure.
  2. Membership is non-transferable.
  3. Participation in the program is voluntary, optional and at the free will of the participant.
  4. The formal enrollment and acceptance into the program is at the sole discretion of Medybiz Private Limited.

Conditions

The program and / or the services offered do not constitute or comprise, advice or intended to be an alternate to consulting a qualified medical practitioner.

Intellectual Property

  1. Unless otherwise stated, Medybiz reserves all copyright in the content and design of the program materials. These materials may not be reproduced, distributed, transmitted, displayed, published or broadcast, either partially or wholly, without the prior written permission of Medybiz, or in the case of third party materials, the owner of the content.
  2. Registered and unregistered trademarks used here in, including but not limited to the word Medybiz, the Medybiz logo, Medybiz Private Limited, and Medybiz Silvercare. You shall not use, alter or remove any of these trademarks, without first obtaining Medybiz’s prior written permission.      

Service Tenure

The service is available for one year from the date it is bought /subscribed for.

Service/Package Description

The Silvercare service offers primary care medical facilities to its beneficiaries. The service consists of the following:

Wellness Package

Comprehensive Health Check
This Health Check is through tests which are done once, only during the enrollment for Silvercare and provides extensive and detailed assessment of a person's present health status. The check includes a medical questionnaire, comprehensive laboratory examination and a consultation with a medical professional. Once the Sponsor buys the service for his/her beneficiary, the Medybiz team contacts the beneficiary and fix up an appointment for the beneficiary to undergo a Health Check. Beneficiary will be responsible for making him/herself available at home/hospital as per the schedule/appointment for his/her Health Check. Medybiz coordinates the collection of samples from home as per suitability/requirement.

Half Yearly Mini Follow-up Health Check
This test is performed only once, after six months and within a period of one year from the date of Comprehensive Health Check test. The Follow-up Mini Health Check is a small package of essential tests that reviews the beneficiary's condition and ensures that he/she continues to enjoy good health. Medybiz coordinates the collection of samples from home.

Customized Health Plan
Based on the results of comprehensive check-up, Medybiz customizes a preventive and promotive health plan to ensure healthy mind, body and soul. The components of the plan may vary from place to place and person to person and is dependent on the beneficiary’s health needs in coordination with the treating hospital or doctor.

Home Visits
Home visits by allied professionals (Inclusive but not necessarily pertaining to Doctor /Nurse/ Counselor/ Physiotherapist/ Occupational therapist/ Dietician/ Delivery executives etc.) for evaluation & regular follow-ups as means of providing holistic health.

Call from Medybiz every week
Emotional support through weekly, pleasantry telephonic calls to assess the general wellbeing and offer help/solutions if required. The telephone call will be made to the beneficiary's residence every week at anytime between 9 AM to 5 P.M on days convenient to Beneficiary. However, if the beneficiary is not available when such a call is made or if the call is disrupted due to faulty line or the phone being faulty or non functioning of the beneficiary's telephone then Medybiz shall not be held liable and such calls shall be deemed to have been made and shall not be carried forward to the next day/week.

Counselor-based Services
Psychological and Dietetic Counseling and advice on lifestyle modifications for healthy living.

Information on Health
Medybiz will provide Health related information validated by doctors via phone calls, articles & mail updates.

Discount on Post Hospitalization and Rehabilitation services
Tailored Post Hospitalization Package & Extensive Care Services as & when required with special 20% discounts.

Discount on Healthcare Products
Discounts on purchase of Medicines & Healthcare Products as applicable by company policies from time to time.

Ancillary services on need basis at nominal additional charges
Credible & skilled ancillary services of Physiotherapy, Diet counseling, Lab Technician, and Delivery of Healthcare products to the doorsteps depending on the individual requirement.

Health status report
Medybiz’s ensures you peace of mind by not only taking complete care of healthcare needs of ‘Silvers’ but also brings to you monthly reports on their health status.  

Cost
The sponsor shall be charged an amount of Rs. 9900 per person per annum (plus one time registration Rs. 100/-) for Silver care basic package and Rs. 15900 per person per annum (plus one time registration Rs. 100/-) for Silver care premium package. Disease-specified charges for Medybiz Home Care Package and Medybiz Post hospitalization package would have to be paid separately in case of availing any of these, the information of which shall be provided after formulation of the need-based personalized disease management plan. These charges are liable for change at the discretion of Medybiz Private Limited from time to time. The charges once paid shall not be refunded.

Acceptance
When once the Sponsor makes payment he/she is deemed to have accepted these terms and conditions and executed an agreement with Medybiz Private Limited.

Customer Support & feedback
The Medybiz Help Desk promptly responds to Queries/Suggestions/feedback from the Customers. The customers can post their queries to support@silvercare.co.in Customers can also reach the Customer Support Division on Phone No.1800 200 0999 during the working days, Monday to Saturday from 9.00 AM (ISD) to 9.00 PM (ISD).

Termination 
 If for any reason, the sponsor/beneficiary desires to stop availing the service he/she may do so in which case he/she is not entitled to any refund of charges paid by him/her. The Medybiz Private Limited may terminate the access of the sponsor/beneficiary to the product and services if the sponsor/beneficiary fails to comply with these terms and conditions or guidelines notified by Medybiz Private Limited hereafter from time to time. Silvercare Service cannot be transferred when it is in usage from one beneficiary to other.

Change of terms and conditions
The Medybiz Private Limited reserves the right to change/modify/alter/reduce/modify the services offered and do all amendments in its terms and conditions as it deem fit at its discretion from time to time.

Disputes redressal
Any dispute arising out of these terms and conditions shall be within the jurisdiction of courts in Mumbai city.

Service is personal
The service conceived under this scheme is exclusively intended for the benefit of the Beneficiary as the case may be and such beneficiary is provided with the services stipulated in the service/package description as mentioned above and nothing more. This scheme does not apply to any person other than the Sponsor/Beneficiary.

Liability
Medybiz Private Limited is not in any way liable for any additional expenses or losses incurred by the sponsor/beneficiary for any reason whatsoever in respect of the services under this Silvercare Plan.
